INFORMED CONSENT FORM:

You are invited to participate in a study of Post-Traumatic Stress Disorder and how it affects employability. We hope to learn about any connection between PTSD and employability.You were invited as a possible participant in this study because you agreed to follow the link on the previous website. IF you decide to participate, you will be asked to complete a short survey and demographic questionnaire that will measure employability. The surveys will take approximately 15 minutes to complete. This survey does not diagnose PTSD.

Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will be disclosed only with your permission. Your IP address will NOT be recorded. If you have any questions, please do not hesitate to contact us.
