Dear Participant,
You are being asked to take part in a research study because you have been identified as a practicing general surgeon. This survey aims to evaluate perceptions and practice patterns among general surgeons in Ontario with regards to the utility of repeat endoscopy prior to surgery for colorectal cancer. It will take approximately 5 - 7 minutes to complete the questionnaire.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point without any consequences. If you decide to participate, please complete all of the questions to the best of your ability. It is very important for us to learn your opinion.
All information obtained during the study will be held in strict confidence. Representatives of the University Health Network Research Ethics Board may look at the study records to check that the information collected for the study is correct and to make sure the study followed proper laws and guidelines. You will not be named in any reports, publication or presentations that may come from this study. Responses will be stored in a locked cabinet at Toronto Western Hospital and no identifying information will be on the questionnaire sheet.
