You have been identified as a CDC/HHS subject matter expert (SME) who has been involved in defining the 15 capabilities for the 2011-2016 PHEP cooperative agreement; and who may also provide training/technical assistance to Public Health and Emergency Preparedness (PHEP) awardees. Therefore, DSLR would like to request your participation in the PHEP program training and implementation needs assessment to better inform the design and development of training and improve technical assistance provided for the 2011-2016 PHEP cooperative agreement.

The purpose of the survey is to assist DSLR to better identify:
a) training needs for awardees and CDC staff related to the PHEP cooperative agreement from your perspective
b) existing training opportunities related to preparedness in your subject matter expertise.
