Hello:

You are invited to participate in my survey on 'Integrity Management and Risk Assessment During the Decommissioning of Offshore Subsea Assets'. This is the final part of my Asset Integrity Management degree and your participation will be greatly appreciated.

The survey is designed to look at the relationship between integrity management, the decommissioning of offshore subsea assets (but not platforms) and identify any areas that could be improved within industry. Although my dissertation is focused on the UK but I am keen to obtain results from all regions as that will provide feedback on issues and problems encountered elsewhere.

There are ten questions in total, made up of multiple choice, open and closed questions. I anticipate it will take approximately 15-20 minutes to complete the questionnaire.

There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point or leave the section blank.

It is very important for me to learn your opinions. Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
