I hereby consent to participate in a research study conducted by Dr. Christopher Collins, Dr. Jeremy Bradbury and Mariana Shimabukuro at the University of Ontario Institute of Technology. This study has been approved by the UOIT Research Ethics Board (file 13-021).
I understand that:
• The purpose of this study is to determine a user’s response to aesthetic quality, ease of comprehension and usefulness of various types of automatic abbreviation techniques.
• The procedure to be used is a web questionnaire that will take approximately 30 minutes to complete.
• After completing the questionnaire, if I choose to provide contact information, I will be entered into a random draw for $50, to be awarded at the end of the study. I will receive no other compensation for my participation.
• If I choose to provide my email address for the compensation draw, this information will be visible to the investigators, but not linked to the other data collected in this study.
• All materials and results will be kept confidential, and, in particular, that my name and any identifying or identified information will not be associated with the data collected.
• During this study, all data is stored on the servers of QuestionPro. After the end of the data collection, data will be downloaded and removed from QuestionPro servers, and stored on an encrypted disk inside Dr Collins’ locked lab. Only the researchers will have access to this data.
• This agreement does not require me to waive my legal rights.
• The data collected during this study may be presented in resulting publications.
Risks
No risks beyond embarrassment of answering questions regarding topics which the participant is unfamiliar with or does not understand.
Benefits
After using this web application the participant may gain some insight on the documents they have uploaded. Results obtained from this user evaluation will contribute to the future development of tools to explore and mange document repositories.
Requesting Withdrawal
Withdrawal at any time from this study is done by exiting the browser displaying the questionnaire before completing the questionnaire. Doing this will delete all previously entered data. Withdrawal will not result in any consequences beyond forfeiting entry into the compensation draw.
Using QuestionPro
An online survey system will be used to conduct the questionnaire and store the data. QuestionPro servers are located in the USA and data collected during this study will be subject to QuestionPro privacy policies: http://www.questionpro.com/help/1.html and the US Patriot Act.
