Dear Soror,
As our chapter continues to grow, it becomes even more important that we provide opportunities for you to give feedback to your leadership team. Please take a moment to complete the survey, which will help us, as a chapter, be the best we can be for the community in which we serve. The survey is anonymous (optional to provide name and contact information) and will take no more than 10 minutes to complete.
The survey will be administered via paper and on-line. Paper administration will begin Saturday, June 20, 2015. Online administration will begin Monday, June 22, 2015. The deadline to complete and receive the paper and online survey is Tuesday, June 30, 2015. Completed paper survey can be turned in by the close of chapter meeting on June 20, 2015, or received in the chapter’s PO Box by June 30, 2015. Results of the survey will be shared with chapter members and we invite your participation in developing our action plan to address areas of concern.
