Dear staff member

I would like to ask you to participate in a gathering of information about the understanding of staff of the difference aspects and procedures associated with the services provided by The Coach House Trust. This is the first part in an on-going effort to ensure that our services are always appropriate to the needs of our service users.

This survey has been sent to all staff within the Coach House Trust. Depending on the department within the Coach House Trust you work in, you may not know much about a given question - this is OK! Please give as much information as you know but it is also help if you simply answer "I don't know".

It will take approximately 15 minutes to complete the questionnaire however it may take longer if your answers are detailed.

Your participation in this survey is completely voluntary. There are no foreseeable risks associated with this survey. However, if you feel uncomfortable answering any questions, you can leave the question blank. It is very important for us to learn your opinions.
