You are invited to participate in our survey to help evaluate the Transitions and Beyond Program for Community Services Coordination Network (CSCN). In this survey, both youth and stakeholders, enrolled or recently discharged, from Transitions and Beyond Program will be asked questions regarding the process and program. It will take approximately ten minutes to complete the questionnaire.
