Dear Trust Index Committee Members,

Thank you for your leadership in developing and implementing the trust survey.  While we await the results of the survey, I would like to ask your feedback regarding your experience on the committee as well as the survey instrument and process.  This will help us make improvements the next time we implement the survey.

Providing feedback is completely voluntary.  If possible, please respond in the next two weeks. I know some of you are very busy right now at the end of the semester and beginning May terms.  If this is bad timing for you to share your feedback, please feel free to get back to me at a time of your convenience.  There is no great rush, other than having the experience fresh on your mind.
