Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. If you have questions at any time about the survey or the procedures, you may contact Fire-Rescue Division Chief Ron DeShong at (954) 602-4833 or by email at the email address specified below.

Tropical Storm Erika threatened City of Miramar from its inception on August 24 through its dissipation on August 29. Although the storm dissipated prior to any major impacts to Broward County, the preparations for the approaching tropical cyclone provided us with some invaluable experience in implementing our preparedness plans. In the spirit of continuous improvement and in order to identify any lessons learned and remedial actions so we are better prepared for the next one, the Miramar Emergency Management Division (EMD) would like to solicit your feedback.

EMD requests that you complete the attached survey by next Tuesday, September 8, 2015, to gather your input. We want to collect this information while it is still fresh and are able to conduct corrective actions as soon as possible to improve future support in pre-storm Emergency Operations Center activities.

Thank you very much for your time and support. Please start with the survey now by clicking on the Continue button below.
