Hi team,
We've decided to reach out and get some input from our employees as we search for a new community-based philanthropy to support in 2014. Following are just a few questions that will help us decide where to spend our time and effort, that can both impact our neighborhoods, and provide connections for our staff. Because you are wellness professionals, we know that you have heart-felt commitments and desires to make a difference. We appreciate your opinions and involvement.

It will take approximately 5-10 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.
