SEICHOU KARATE CHILDREN'S WITHDRAWAL SURVEY

We’re sorry that your child will be leaving Seichou Karate.

As you know children enroll three times a year at Seichou Karate: from January to the end of May, from June to the end of August and from September to the end of December. Enrollment in our children’s programs is a commitment to complete the three, four or five-month enrollment period for which your child is currently registered. As such, your commitment to pay us runs to the conclusion of the current enrollment period.

To formally withdraw your child, please complete this questionnaire.

Our staff will use your responses to improve the quality of our programs. We will not divulge them to any other person or entity.
