NOTE: It is Eau Claire United Soccer Club’s policy to share a synopsis of the coaching evaluations received from the parents on the team with the coaches. Evaluations will address performance areas in need of attention or confirm satisfactory or exemplary performance.

Evaluation Guide (please circle appropriate rating):

1 = Low Performance

2 = Low Average Performance

3 = Above Average Performance

4 = High Performance
