Contact Information - Form Fields

The Form fields option is available with contact information question settings. QuestionPro allows you to add or remove the form fields of the contact information question based on the details that you wish to capture from the respondents.

By default the following form fields can be seen when you add contact information question:

• First name
• Last name
• Phone number
• Email address

Consider that you want to ask for the respondent’s mailing address as well.

• Go to settings, under form fields switch on the toggle for mailing address.
• The following form fields will be added to the contact information question:
  • Address line 1
  • Address line 2
  • City
  • State
  • Zip

  You can also remove any of the above mentioned form fields (e.g. address line 2) by switching off their respective toggles.

For phone number, QuestionPro supports four types of format:

• United States / Canada
• International
• India mobile
• United Kingdom

You can also change the default country under phone number format. To change the default country:

• Click on the phone number drop-down and select format as shown in the above image.
• Under format, the default country is the United States/Canada.
• Click on the format drop-down to view the list of countries and select your country.
